The world of medicine is facing a silent yet deadly threat: antifungal resistance. While the battle against bacterial and viral infections has been a priority, a group of infectious disease experts is now sounding the alarm over the growing resistance to antifungal drugs. This issue is not just a medical concern but a call to action for global strategies to combat antimicrobial resistance (AMR).
In a recent publication in Nature Medicine, over 50 researchers from around the world, led by Paul Verweij of Radboud University Medical Centre, have outlined a five-step plan to address this emerging crisis. The plan emphasizes the urgent need to integrate antifungal resistance into global health strategies, focusing on awareness, surveillance, infection prevention, optimized therapy, and investment in new diagnostics and medicines.
One of the key concerns highlighted is the rise of drug-resistant fungi, such as Candida auris and Aspergillus fumigatus. These infections can be life-threatening, especially in vulnerable patients with compromised immunity. For instance, Candida auris can cause serious bloodstream infections, with one in three patients not surviving. Aspergillus fumigatus, an inhaled infection, is becoming more prevalent in intensive care units (ICUs), particularly in patients with severe influenza.
The experts argue that the current global strategies against AMR, which primarily focus on bacteria and viruses, are not giving antifungal resistance the attention it deserves. They call for near-term milestones, equitable financing, and stewardship that integrates clinical, agricultural, and environmental policies. This includes support for pragmatic trials, technology transfer, and local manufacturing, especially in low- and middle-income countries (LMICs).
The pharmaceutical industry's retreat in recent decades has resulted in only a handful of new antifungal agents being introduced to the market since the WHO's Global Action Plan on AMR was first drawn up in 2015. Recent launches include Rezzayo (rezafungin) by Cidara/Melinta in 2023, Vivjoa (oteseconazole) by Mycovia Pharma in 2022, and Brexafemme (ibrexafungerp) by GSK/Scynexis in 2021. These developments are crucial but insufficient to combat the growing resistance.
The experts' five-step plan is a call to action for the global health community. It emphasizes the need for a holistic approach to antifungal resistance, integrating clinical, agricultural, and environmental policies. The plan also highlights the importance of equitable financing and support for pragmatic trials and local manufacturing.
